By booking a session with Sarah you understand and agree to the following statement:

By booking a session with practitioner Sarah Haggerty, I consent to receive services, and/or give consent for my minor child to receive services. I understand these services are intended to promote relaxation and support the body’s natural ability to restore balance.

I acknowledge that these services are not a substitute for medical care, and that no diagnosis will be given and no medication prescribed. I agree to continue receiving regular medical care as part of my overall health plan.

I understand that participation is voluntary and that I (or my child) may stop at any time. I also understand that I or my child may experience temporary “healing reactions” (such as fatigue, emotional release, or vivid dreams) within 24–48 hours after a session.

I understand that any information shared during sessions is confidential and will not be released without written permission. I consent to my or my child’s experiences being referenced anonymously for educational purposes.

I confirm that I am 18 years of age or older, or the legal parent/guardian of the minor receiving services.
